The determination
What was decided. The decision itself, attached to the thing it was about, rather than an email that mentions it.
The evidence
What was in front of the person when they decided. The drawing, the quote, the clause, the reading, the alternative that was priced and not taken.
The signature
Who decided, in what role, on what date. A question years later has a person's name on it.
One entry is two facts at once: a fact about the job and a fact about your organization, joined by the determination DEC-3141-06 and its evidence. Anchored to the element itself, so the record survives the vendor, the system and the people who were there when it was made. The evidence sits between them. Your tools will change four times in the next twenty years. The record does not have to change once.
The record keeps what you used to think.
When something turns out to be wrong, the correction is added and the original stays underneath it. Nothing is edited and nothing is deleted. The record cannot quietly become a different record.

An entry cannot be altered without breaking the link to the one after it, so what the record said in March is still what it said in March. Open a superseded entry and it is all still there, evidence included.

An owner, an auditor, a surety, a claims lawyer. When someone asks why this was approved and on what basis, the answer takes minutes rather than weeks.

Everything decided in design and delivery arrives with the asset, so whoever runs it for the next forty years inherits the reasoning and not just the drawings.
